Guardian Registration Instructions

REGISTRATION IS BY APPOINTMENT ONLY. Upon completion of Sections 1 & 2 please make an appointment by calling (973)822-3855 X2006. Registration of all children coming into the Florham Park School District will take place in Ridgedale Middle School (Directions). If the required documents are not presented, admittance may be delayed.

REGISTRATION HOURS: 9am–1pm on Wednesday ONLY during the school year. In-coming students will begin classes no sooner than the following Monday. Summer Registration hours are as follows: 10:00AM - 1:30PM on July 8th and 9th, and August 12th, 13th, 26, and 27th.

SECTION 1. Prior to registration:

 BEFORE REGISTERING TO ENTER A FLORHAM PARK PUBLIC SCHOOL WE MUST HAVE IN OUR POSSESSION:

  1. Student Registration/Information Form (download)
  2. (Student) Health Information Form (download)
  3. Original Birth Certificate (legal proof of age)
  4. Transfer Card for New Jersey in-state transfer (from former school)
  5. Proof of residency (lease or ownership documentation)
  6. Health records containing immunization records
  7. Information concerning special services (speech, I.E.P., etc.)
Signed Release of Records Permission Form (Done at the registration interview)

Has the student taken a foreign language? (Ridgedale Middle School students must take either French or Spanish if in 7th or 8th Grade. Sixth graders take Spanish). If "yes" indicate language __________ # of years ________.

    (Gr. 3-8 Gr. 2 violin only) Is the student interested in playing a musical instrument. If "yes" indicate instrument __________ # of years ________.
    Grades 6-8) Is the student interested in an organized sport (soccer, cross country, basketball, baseball/softball, wrestling, bowling or tennis)? Link to athletic forms.
    (Grades 2-8) Is the student interested in joining the chorus?
